VOIP Passthru / SIP Trunks

Can't get the router to pass thru the VOIP calls. Connecting directly to the Cable Modem/Router with a static IP... no problem. All calls go thru, however when I introduce the Netgear and the port forwarding neccesary.... SIP services will not come online. Any Idea's..... ?? I cannot specify UDP 5060. Only 5060 to 5060 to the internal IP Address assigned. All pings are good, so I know it it connected.
